Certified Family Nurse Practitioner

Summary/Objective

Family Health Services is a non-profit, Federally Qualified Health Center (FQHC) providing quality, affordable health care to all. The Certified Family Nurse Practitioner provides medical services to the patients of Family Health Services in acute care and the clinical office setting in a holistic approach. We offer a competitive salary and benefits package including malpractice insurance coverage. Generous student loan repayment is also available through the NHSC Loan Repayment program, based on a 2-year full-time commitment.

Candidate must hold a diploma from an accredited School of Advanced Practice Nursing; licensed to practice as a Nurse Practitioner in the State of Ohio with current Nurse Practitioner Certification and a desire to serve the community of patients we serve. Essential Functions

The Certified Family Nurse Practitioner provides medical services to the patients of Family Health Services in acute care and the clinical office setting in a holistic approach. Essential Functions

1. Completes patient history and physical.

2. Documents all patient care provided according to policy and Standard of Care Agreement.

3. Formulates and directs plan of care for the patient as according to Standard of Care Agreement.

4. Refers to other physicians, specialists as needed.

Psychiatric Mental Health Nurse Practitioner

Summary/Objective

Family Health Services is a non-profit, Federally Qualified Health Center (FQHC) providing quality, affordable health care to all. The Psychiatric Mental Health Nurse Practitioner will provide advanced nursing care for patients with psychiatric disorders and may provide psychotherapy under the direction of a psychiatrist.

Duties/Responsibilities:

  • Diagnoses psychiatric disorders and mental health conditions. Distinguishes between physiologically and psychologically based disorders and makes appropriate diagnoses.
  • Assesses patients’ mental and physical status based on the presenting symptoms and complaints. Conducts individual, group, and family psychotherapy for those with chronic or acute mental disorders.
  • Documents patients’ medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, and outcomes. Writes prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements. Monitors patients’ medication usage and results.
  • Evaluates patients’ behavior to formulate diagnoses or assess treatments.
  • Educates patients and family members about mental health and medical conditions, preventive health measures, medications, and treatment plans.
  • Collaborates with interdisciplinary team members including psychiatrists, psychologists, and nursing staff, to develop, implement, and evaluate treatment plans.

Required Skills/Abilities:

  • Ability to diagnose and treat simple and complex psychiatric and mental health problems from adjustment disorders to serious mental illnesses such as schizophrenia, bipolar illness, major depression, and anxiety disorders.
  • Strong skills to manage, coordinate, and evaluate the care and education of the patient and family along the health care process.
  • Ability to manage patients who need to be closely monitored.
  • Trained to adhere to medications and therapies with complex protocols.

Job Description:

  • Expertise in handling sophisticated medical equipment.

Education/Experience:

  • Bachelor’s degree in nursing and an advanced degree from an Advanced Practice Psychiatric Nursing (APPN) graduate nursing program required. APPN graduate education must include the study of basic science, (i.e., pathophysiology, advanced health assessment, brain and behavioral correlates), advanced psychopharmacology and psychotherapeutic techniques; followed by clinical rotations in outpatient mental health settings, hospitals, or residential care settings.
  • Certification by the American Nurses Credentialing Center (ANCC) in psychiatric mental health advanced practice nursing as the area of specialization required.
  • Documented compliance with any state statutes and regulations governing advanced practice psychiatric mental health nursing required.

Psychiatrist

Dentist

Summary/Objective

The Dentist is responsible for ensuring the provision of quality of dental services delivered by the staff of Family Health Services (FHS). The Dental Director has chosen to delegate specific responsibilities of the position, aside from dental physician peer review and discipline to the Dentist. This position will assume responsibility for the delivery, coordination and supervision of dental services provided at FHS, and abides by FQHC policies, federal, state, and local regulations within the Dental Department. The person provides direct dental care.

FHS is a National Health Service Corps (NHSC) approved health care site. This allows the dental provider (DDS/DMD) to be eligible to apply for the NHSC Loan Repayment Program with an award amount up to $50,000 for Full-Time status employees and a 2-year Service Commitment.

Essential Functions

  • Routinely meetings with Dental Director to review dental operations.
  • Assists the Dental Director in carrying out the dental care plan.
  • Meet with patients to discuss and treat dental concerns, performing regular cleanings and other preventative procedures, and establish a plan for better dental hygiene.
  • Performing dental procedures, such as extractions, root canals, and filling cavities.
  • Correcting bite issues and overcrowding.
  • Applying helpful agents to teeth, such as sealants or whiteners.
  • Prescribing medications for dental problems, such as pain medications or antibiotics.
  • Giving clients sedatives or anesthesia prior to administering treatments.
  • Ordering diagnostic measures, such as x-rays, models, etc.
  • Using tools, such as drills, probes, brushes, or mirrors, to examine and treat teeth and mouth.
  • Keeping records relating to the oral health of patients and the treatments given to them.
  • Managing and communicating with other staff members to provide care to patients.
  • Exhibits a caring, communicative, and positive interaction with patients and within the total organization.
  • Represents the organization at various community and/or business meetings, committees, and task forces, promotes existing and new programs and/or policies.

Dental Hygienist

Summary/Objective

The Dental Hygienist is responsible for the direct delivery of oral hygiene and appropriate preventive services to dental patients of the center. The Dental Hygienist will also be responsible for providing appropriate oral health information to FHS participants and staff, including outreach events.

Essential Duties & Responsibilities:

  • Assists in the oral health management of all clinic patients.
  • Obtains baseline oral hygiene assessment data based upon accepted principles of oral hygiene and periodontal care.
  • Records patient-hygienist transactions as they occur in the patient’s dental record so that the dental record accurately and completely reflects the nature of the contact, the condition of the patient and the care or treatment provided.
  • Provides oral health education and appropriate individual counseling for all center dental patients.
  • Provides clinical oral hygiene services including dental prophylaxis, scaling, sealant application and fluoride application consistent with accepted professional practices and standards and in compliance with applicable state law and the center’s clinical protocols. Works with Dentist to assist in coordination of care and referral for patients receiving preventive services.
  • Performs independent procedures as delegated and directed by a dentist in accordance with state regulations and law as well as Oral Health Department protocol and directive.
  • Assists in the maintenance of all Oral Health Department areas in compliance with department directives and policies as well as center policies and procedures relative to infection control, exposure control and safety issues.
  • Receives and places necessary telephone calls consistent with professional matters, clinic business and patient care of the Oral Health Department.
  • Assists with various clinical and administrative functions of the center as appropriate and time permits.
  • Ensures the proper disposal of all contaminated or potentially contaminated materials in accordance with Oral Health Department directive and center policy as well as state federal regulation(s).
  • Complies in full with the center’s Exposure Control Plan.
  • Travels when necessary to meet operational needs.
  • As directed by a supervisor, performs other related and/or necessary tasks to achieve organizational and programmatic goals and objectives.
  • Responsible for personal compliance with all applicable federal, state, local and center rules, regulations, protocols and procedures governing the practice of dental hygiene and the clinical provision of dental hygiene services as well as those relating to, but not limited to, personnel issues, work place safety, public health and confidentiality.

Graduation from an accredited school of dental hygiene is required. Ability to maintain appropriate clinical privileges is required. Must have unrestricted license to practice Oral Hygiene in Ohio and current CPR (BLS) certification for this position.

Position requires a comprehensive knowledge of oral hygiene care in outpatient settings. Knowledge of the principles and practice of modern oral hygiene and periodontal patient care is necessary. The Dental Hygienist must be cognizant of the public health, social and economic factors which impact on the overall health, and more specifically the oral health of their patients. The Dental Hygienist must be able to creatively work with other health care professionals from a variety of disciplines to achieve maximal program effectiveness and patient benefit. Summary/Objective

Meets with patients, their families, guardians or agency representatives and completes behavioral health diagnostic assessment. Provides psychotherapy for individuals who qualify for services on site.

Essential Functions

  1. Completes behavioral health diagnostic assessments.
  2. Creates positive relationship with patients, their families, guardians or agency representatives.
  3. Provides psychotherapy for individuals, couples, and families who qualify for services on site.
  4. Advocates and refers individuals with serious mental illness and/or substance abuse/addiction to community agencies for treatment.
  5. Networks with local social service agencies and educational agencies to remain knowledgeable about available services and the referral process for those services.
  6. Provides crisis intervention services on site, as needed.

Medical Assistant – FT/PRN

Summary/Objective

The Medical Assistant performs duties essential to the patient visit and other duties as assigned.

Essential Functions

  1. General office skills.
  2. Disseminate information by telephone, mail service or e-mail.
  3. Performs other duties as assigned.
  4. Triage responsibilities.
  5. Assist providers in patient diagnosis by gathering patient history, obtaining vital signs, chief complaints, verifying medications and allergies, ensuring patients are properly dressed/undressed for exam. Afterwards, review the chart for presence of information that the provider might need during the visit (labs, consults, ER reports).
  6. Must have an understanding of legal and ethical issues.
  7. Scheduling and managing patient appointments and referrals with required documentation.
  8. Clean, prepare, and sterilize instruments.
  9. Performs electrocardiograms, phlebotomy, and laboratory procedures as directed by health care provider, including but not limited to pregnancy tests, strep tests, urine dips, hemoglobin, and blood sugar testing.
  10. Perform routine administrative and clinical tasks such as aerosol treatments ordered by a physician.
  11. Updating and filing patient’s medical records.
  12. Collect, prepare, and perform basic laboratory specimens and tests.
  13. Instruct patients about medications and special diets.
  14. Prepare and administer medications as directed by a physician.
  15. Telephone prescriptions to pharmacy.
  16. Ensure exam rooms are clean, stocked, and ready for patients.
  17. Remove sutures and change dressings.
  18. Observe and follow required safety protocols, including handling and disposal of sharps, use of personal protective equipment, exposure to blood borne pathogens, and general safety of employees and patients.
  19. Assist other team members as needed.
  20. Attends staff meetings.

Work Environment

This job has frequent exposure to dust, constant noise, vibrations and hands in water. This job has occasional intermittent noise, outside exposure, excessive heat, cleaning agents, excessive cold, dampness, excessive humidity, dry atmospheric conditions, and animals.

Physical Demands

Physical demands include lifting up to 30 pounds daily, carrying up to 30 pounds daily, 1-2 hours of standing at a time, 5-10 minutes of walking at a time throughout the day, sitting 15-30 minutes at a time. Color distinction is necessary, and no greater loss of hearing than 40dB average for 500, 1000, 2000 H.

This position requires the ability to multi-task and prioritize responsibilities, handling patient requests, placing outbound calls to patients, and handling all administrative tasks with a positive attitude.

Essential Functions

    1. Welcomes patients and visitors in person by greeting them in a timely manner
    2. Receives phone calls and directs questions to appropriate personnel according to policy.
    3. Documents phone calls and conversations in record as needed according to policy.
    4. Effectively communicates with both internal and external customers to ensure smooth operation of the clinic.
    5. Optimizes patients’ satisfaction, provider time, and clinic space utilization by efficiently scheduling appointments according to policy
    6. Verifies insurance benefits and eligibility prior to scheduled appointments using various web portals
    7. Pre-registers patients and reminds patients of appointments via telephone the day prior to scheduled appointments.
    8. Registers and checks patients in for their appointments in a timely manner
    9. Maintains patient accounts by accurately entering data such as demographics and insurance information in the electronic health records system
    10. Scans documents into EMR as appropriate, including but not limited to patient’s photo ID, insurance card, intake paperwork, and financial information related to the FHS Sliding Fee Scale
    11. Faxes, scans, files, and mails documentation as necessary
    1. Collects and posts patient payments utilizing office software, provides patient receipts for payments
    2. Ensures kiosks are functioning for patient check-in
    3. Assists patients with completing medical information forms and obtains signatures for consent according to policy.
    4. Completes return to work/school slips in coordination with nurse or provider.
    5. Provides a positive patient experience by anticipating the needs of patients, answering patient’s questions, and maintaining a clean and presentable front office and reception area.
    6. Assists in maintaining office equipment.

This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor, suggestion box, and any suspected unethical, fraudulent, or unlawful acts of practices.

Required Education and Experience

High school diploma with knowledge of computers and data entry is required.

Preferred Education and Experience

Certificate/Degree or equivalent experience in EMR or related field. Minimum one year of clerical experience

Work Environment

This job operates in a health care office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This job has frequent exposure to dust, constant activity and noise, hands in water, excessive heat, cleaning agents, excessive cold, dampness, excessive humidity, and dry atmospheric conditions.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Must have the ability to travel independently throughout the facility to access information from other departments. Must have the ability to sit for long periods of time. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
